Continuing Care

The Issue:

Canadian seniors and people with disabilities deserve the care they need to live with dignity and respect. Unfortunately, due to a fractured system of support and the absence of a national strategy and standards, many Canadian seniors and people with disabilities are falling through the cracks when it comes to the continuum of care (assisted living, palliative care, home care, long-term care, acute care, residential care, pharmacare, primary care). This has to change.
